General Forms
Below you will find forms which are made available as a convenience to players and coaches who must complete a form, but do not have access to the member's only website.
Forms are downloaded in either Adobe .pdf format, Microsoft Word or in Microsoft Excel.
#R002-Youth: Youth Player Registration & Medical Treatment Authorization Form - This is the form which is required of all players who are registering through their club with US Club Soccer. As an alternative to receiving this form from your club, players or parents may download the form from this site, complete it, and submit it to your club.
#R003-Adult: Adult Player Registration and Waiver Form - This form is required of all adult players (U-20 and over) who are registering through their club with US Club Soccer. Instead of uploading a Proof of Birth document with each player's registration, please upload this completed form for each player. (Registro para Jugadores Adultos Y Beneficios en Español)
Staff Background Check Application - Have each coach and administrator who work with players complete this online staff background check. This replaces the paper risk management form and is the only form necessary to submit for a staff member
#R005-Adult League Application - Use this form as a templae to apply to US Club Soccer for adult league sanctioning. Please download it, fill in the finformation and return it via email to leaguesupport@usclubsoccer.org. After reviewing your information, we will contact you to discuss your plans in greated detail and perhaps ask for further information. Email the completed application to leaguesupport@usclubsoccer.org.
Tournament Insurance - Go to this page which has all the information you need regarding tournament insurance.
#R0012 Rec League Application - Use this form as a template to apply to US Club Soccer for recreational league sanctioning only. After reviewing your information, we will contact you to discuss your plans in greater detail and perhaps ask for further information. Note that recreational leagues register their players via an Excel roster, and are not issued player cards.
#R003 Rec/Intramural Player Roster - If you are a recreational/ intramural team or club, and have received approval to register with US Club Soccer, please utilize this roster form to submit your player information to us. Note this is in lieu of online registration, and the players will be covered by insurance, but will not receive player cards. A medical authorization is needed, but no proof of birthdate. Rec player fees are $8 per player. The medical form and fees are sent to US Club Soccer.
